1906 Jan 13 Death Certificate (#1047) for son John Fisher indicates both parents and their birth locations [Daniel, Berks, PA and Martha New Jersey]

In the book, A Human Life, An Autobiography with Excursuses, by Daniel Webster Fisher, (written 1909), page 21, he states that he is the son of Daniel Fisher 1791 Pa and Martha Middleswarth Fisher born 1794 New Jersey. Daniel W Fisher says that his father was a native of Berks County, Pa, and was of German descent. Daniel W Fisher says his father spoke German only when someone German would come to visit, but he wrote in German. Daniel Webster Fisher said he had a German family bible, and Daniel Fisher 1794's father was Michael Fisher, and Michael Fisher's father was John Fisher. Also states that his mother was a Middleswarth and born in Bethlehem Township, Hunterdon County, New Jersey, and her fatherJohn Middleswarth was a Revolutionary War soldier, and her people came from Holland. Her brother was Ner Middleswarth, the PA Congressman.Her father, John Middleswarth was in the Revolutionary War at theBattle of Princeton.
